40-49 Health Assessments

The Madison Medical Practice Hornsby is currently undertaking a government initiative targeted at all patients aged 40 to 49 years of age. This comprehensive assessment allows us to take a deeper look into your lifestyle, family and personal history to identify risk factors for developing a chronic illness.

A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.

Topics will be discussed during your health assessment:

  • a family history of chronic disease, including stroke, heart disease, diabetes, cancer and osteoporosis
  • lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol use
  • biomedical risk factors, such as high cholesterol, high blood pressure, impaired glucose tolerance or obesity
  • your mental health and feelings of depression or anxiety

The health assessment will initially be with our practice nurse. This usually takes around 30 minutes and then your doctor will see you straight after for a further discussion. At this time, we will discuss recommendations to help you maintain or improve your health and to prevent health issues in the future.

If you’re eligible, there will be no cost to you.

Over 75 Health Assessments

You may be aware that once you are over 75 you are eligible for a free annual health assessment. Annual comprehensive health assessment provides an opportunity for your doctor at the Madison Medical Practice Hornsby to undertake an in-depth assessment of your health.

We will take a detailed look at things such as your risk of falls and safety, memory capacity, nutritional status, risk of cardiovascular disease, vision and hearing, bladder and bowel function, immunisation status, and medications both prescribed and over-the-counter.

The health assessment will initially be with our practice nurse. This usually takes around 45 minutes and then your doctor will see you straight after for a further discussion. At this time, you can discuss any recommendations to help you maintain or improve your health and to prevent health problems in the future.

This is at no cost to you as an annual review is completely covered by Medicare.  All it takes is an hour of your time.
